But even without the testing issues: my working thesis is that the infection cycle (from infection to infection) is ~ 20 days or two weeks. I also think that the Index case likely only infected his wife, and the doctor. And maybe one or two more. The cases we are seeing now (are the third generation of humans infected). This would give us roughly an r0 of ~ 3, which is in line with other outbreaks.

I did some more reading on the El Bolson outbreak. This outbreak virus is more similar to what was tested from the current one. Its all in the same family of Andes-Sur but there are various Clades or sub families. The clade 2 cause h2h spread but limited like the WHO - mothers breastfeeding give it to their babies or people within the same household. The el bolson outbreak spread to Buenos Aires in the 90s so its not fair to say that this virus hasnt been in a city. Even though they couldn't trace every contact, the overall outbreak impacted 77 people and 30-something died, including a newborn. The case fatality rate was around 50%.
They believe it was a bad year for the rats so there were multiple zoonotic jumps in 95-96. Another thing noted is that the fatality rate was higher among the first h2h transmissions- possibly because no one was taking precautions. There was spread through social contact but the outbreak fizzled out even in Buenos Aires without a massive quarantine.
